Baystate Health Community Health Needs Assessment (2025)

The 2025 CHNA updates the prioritized community health needs identified in the 2022 CHNA report, which focused on three areas: the social and economic drivers of health, barriers to healthcare access, and health behaviors and outcomes. The federal government requires nonprofit hospitals and insurers to conduct a Community Health Needs Assessment (CHNA) every three years to better understand the health needs of their communities. These reports don’t just help hospitals and insurers—they also provide valuable information that many organizations use to plan programs and services.
